The "public interest" is established in many laws as a ground for compulsory licenses. The concept of what constitutes the "public interest" varies amongst countries and over time. It will be the task of the courts or administrative authorities to judge when the public interest is to be ensured or protected via the granting of a compulsory license. H2 Blocker Exposure as a Risk Factor for Depression Dear Sir: A small literature of case reports describes the emergence of depressive episodes in patients treated with H2 blockers 16 ; , but empirical investigations have thus far failed to confirm the association 79 ; . We have conducted a series of casecontrol studies concerned with drug exposures as possible risk factors for clinically diagnosed depressive disorders among hospital patients. A detailed description of the methods employed in this series of studies may be found in a previous publication 10 ; . These studies utilized electronic case-summary data from the Calgary General Hospital these data are maintained by most Canadian hospitals for submission to the Canadian Institute for Health Information ; . The data were used to identify potential cases subjects with ICD-9 codes indicative of clinically diagnosed depressive disorders ; and controls for the casecontrol studies. Selection of subjects was restricted to diagnostic categories constituting an indication for treatment with H2 blockers: duodenal and gastric ulcers, gastritis and duodenitis, reflux esophagitis, and Zollinger-Ellison syndrome. These subjects' clinical records were then reviewed, and an attempt was made to confirm the casecontrol status. No attempt was made to apply formal diagnostic criteria during the chart reviews, but confirmation of a clinical diagnosis was required. Drug exposure information and some other clinical data employment status, marital status, nonpsychiatric diagnoses, past history of depressive episode, family history of depressive episode, and clinical diagnoses of substance use disorders ; were recorded when available. The initial sample consisted of 324 subjects: 120 potential cases with depressive diagnoses ; and 204 potential controls randomly selected from 1685 subjects with a potential indication for H2 blockers as a discharge diagnosis, but without a psychiatric discharge diagnosis. All of these patients were discharged from the hospital between April 1990 and October 1993. Upon review of the charts, the diagnostic designations contained in the electronic data were confirmed for 304 93.8% ; of the 324 subjects, so that the final sample consisted of 107 cases and 197 controls. Of the cases, 21 19.6% ; were taking an H2 blocker prior to their admission to hospital, while 29 14.7% ; controls were taking an H2 receptor blocker. The odds ratio OR ; was 1.41, with exact 95% confidence limits that crossed the null value of 0.72 to 2.74. One hundred and fifty-two subjects were newly exposed to H2 blockers while in hospital. These included 36 of the 107 cases 33.6% ; and 116 of the 197 controls 58.9% ; , leading to an OR 0.35, and exact 95% confidence limits of 0.21 to 0.60. Other risk factors for depressive disorders had significantly elevated odds of exposure among the cases: female subjects OR 2.42, 95% CI 1.45 to 4.05 ; , subjects with a recorded past history of depressive disorder OR 9.88, 95% CI 4.49 to 23.33 ; , and subjects whose marital status was recorded n 259 with available data ; as separated, divorced. 24 ; radiation therapy is an option in the patient who cannot tolerate medications and is not a surgical candidate and cromolyn and keftab, for instance, medications.
