Based upon the foregoing facts and reasoning, the administrative law judge conclude that the provisions of the Act permit and require the transfer of cannabis from Schedule I to Schedule II. The judge realizes strong emotions are aroused on both sides of any discussion concerning the use of cannabis. Nonetheless it is essential for this Agency, and its Administrator, calmly and dispassionately to review the evidence of record, correctly apply the law, and act accordingly. Marijuana can be harmful. Marijuana can be abused. But the same is true of dozens of drugs or substances which are listed in Schedule II so that they can be employed in treat, for example, combivent 2 puffs.

Currently there is no evidence that putting large groups of otherwise healthy Canadians on antivirals in order to prevent influenza i.e., prophylaxis ; will slow or stop the spread of a pandemic; however, prophylaxis with antivirals may play a key role in maintaining critical services i.e., preventing infection in and providing reassurance to people caring for individuals with influenza as well as workers in critical industries ; until a vaccine becomes available. Both the 2004 and 2005 OHPIP listed preliminary groups of people who, based on their health status or their role during a pandemic, would be the first to receive antivirals for treatment or prophylaxis. Ontario has removed those lists from the 2006 OHPIP and will develop a provincial policy on the use of antivirals for prophylaxis based on the national policy under development as of February 2007 ; and in accordance with the ethical framework for decision-making. This will help ensure a consistent approach to using antivirals for prophylaxis across all provinces and territories, which will lead to stronger public confidence and morale. With regard to antivirals for treatment, the Ontario government is committed to providing treatment for individuals who become ill during an influenza pandemic and will maintain an antiviral stockpile large enough to treat 25% of the population, because combivent cost.

Schoon, PhD, Canine Unit, Netherlands National Police, PO Box 530, Nunspeet, 8070 AM, Netherlands; and Kenneth G. Furton, PhD, Florida International University, 11200 SW 8th Street, Department of Chemistry and Biochemistry, Miami, FL 33199 After attending this presentation, attendees will learn about the effects of biological and chemical influences from hand odor samples on alerts produced by human scent identification canines. This presentation will impact the forensic community and or humanity by providing a better understanding of the correlation between volatile organic compounds and the human and non-human compounds present in hand odor samples and canine alerts. Human scent identification line-ups are frequently utilized in European countries such as the Belgium, Bulgaria, Denmark, Finland, France, Germany, Hungary, Lithuania, Netherlands, Poland, Russia, and Slovakia. They establish an association between a suspect and an object or location based on canines matching human scent collected from a crime scene to scent collected from the hands of a suspect. Scent identification line-ups are possible as persons have distinctive odors and canines are capable of discriminating between odors. Many theories have been put forward to explain the production of human odor as it is complicated process which is not yet fully understood. The basis on which the dogs are producing an alert to human scent is also yet to be determined. The human body produces odors made up of a variety of organic compounds such as fatty acids, alkanes, aldehydes, ketones and esters. Human scent has been defined as the volatile organic compounds which are present in the headspace of a scent sample. However, other substances may also contribute to human scent. Solid phase micro extraction gas chromatography mass spectrometry SPME-GC MS ; is an analytical technique which has been used for the extraction of volatile organic compounds which are present in the headspace of various forensic samples such as drugs and explosives. SPME-GC MS has been successfully used for the extraction, separation, and identification of the volatile organic compounds which are present in the headspace of scent samples and cyclobenzaprine.
Patients given 1 g kg AMG 531 and 3 of 8 38% ; patients given 3 g kg reached the 50, 000 platelet L blood target compared with 15% for placebo. Two patients receiving 3 g kg were forced to discontinue after exceeding 450, 000 platelets L. Notably, no anti-AMG 531 or anti-TPO antibodies were detected after treatment. Patients received up to six weeks of treatment. AMG 531 is given in weekly subcutaneous injections in doses individualized for each patient. At the 2005 ASH meeting in Atlanta, AMGN presented data from a 48-week, open-label, follow-up study showing that 29 of 34 patients 85% ; given AMG 531 achieved a platelet response, defined as a doubling of the baseline platelet count and at least 50, 000 platelets L blood, also without neutralizing antibodies. Eltrombopag is a biphenyl hydrazone, and AMG 531 is a platelet growth factor peptibody engineered with Fc receptor. Both are in Phase III testing for ITP. According to GSK, its small molecule may have less potential for causing an immune system reaction than large protein molecules such as AMG 531. Indeed, companies originally tried to restore platelets by giving recombinant TPO, but this resulted in the formation of neutralizing antibodies against both the molecule and endogenous TPO. The next step was to develop TPO-like agonists that mimic the interaction between natural TPO and its receptor, triggering pathways that stimulate the proliferation and differentiation of megakaryocytes, the bone marrow cells that give rise to blood platelets see BioCentury, Jan. 31, 2005 ; . Janet Nichol, AMGN's global development leader for AMG 531, told BioCentury that during the discovery process for the peptibody, the company specifically screened for a sequence not similar to that of natural TPO, in order to avoid immune responses. Nichol also suggested subcutaneous injection of AMG 531 might be an advantage in ITP, a disorder which has a variable clinical course and for which treatment must be highly flexible to account for fluctuating platelet levels in individual patients. In addition, she would not exclude the potential for an oral formulation in the future. Eltrombopag, which was in-licensed from Ligand Pharmaceuticals Inc. LGND, San Diego, Calif. ; , is in Phase II trials in thrombocytopenia, both associated with HCV and induced by chemotherapy. The pharma company expects data to be available by year end from the Phase III 773B trial for ITP, which recently completed enrollment of about 160 patients. AMG 531 is in two Phase II trials to treat myelodysplastic syndrome MDS ; and chemotherapy-induced thrombocytopenia. The two Phase III trials for ITP, for which AMG 531 has Fast Track designation, are expected to be completed by year end, but AMGN Thousand Oaks, Calif. ; would not say when data might be available. -- Alexei Ku.

Given its characteristic clinical picture and very good prognosis when recognized and treated, potential exposures can readily be managed by observation and treatment as clinically needed. Risk-based Medical Management of Possible Exposures Low-credibility exposure situations and situations with possible cutaneous exposure If no clear-cut exposure e.g., patient was in New York, now has cold symptoms and is worried ; , provide reassurance to the patient about the rarity of infection without known exposures. We do not recommend collecting a nasal swab or blood for a serologic test to try to confirm that there is no evidence of exposure to anthrax. We do not recommend prescribing prophylactic antibiotics in these situations. If the only potential exposure to a powder suspicious substance is cutaneous the usual situation with finding powder on a surface, opening a letter with powder in it ; , provide advice on what to look for red spot - papule - vesicle - black center over several days to a week ; , reassure them that cutaneous anthrax can be readily diagnosed and easily treated. We do not recommend collecting a nasal swab or blood for serology in the absence of a skin lesion, nor do we recommend prescribing antibiotic prophylaxis. This situation is analogous to the rabies situation of having a provoked bite from an animal that is highly unlikely to have rabies - e.g., squirrel that bites finger.

Disease and frequently enter the bloodstream but rarely cause IE with fewer than 120 cases reported.41 Viridans group streptococci are antagonistic to periodontal pathogens and predominate in a clean, healthy mouth.42 Few published studies exist on the magnitude of bacteremia after a dental procedure or from routine daily activities, and most of the published data used older, often unreliable microbiologic methodology. There are no published data that demonstrate that a greater magnitude of bacteremia, compared with a lower magnitude, is more likely to cause IE in humans. The magnitude of bacteremia resulting from a dental procedure is relatively low less than 104 colony forming units of bacteria per mL ; , similar to that resulting from routine daily activities, and is less than that used to cause experimental IE in animals 106-108 colony forming units of bacteria per mL ; .20, 43, 44 Although the infective dose required to cause IE in humans is unknown, the number of microorganisms in blood after a dental procedure or associated with daily activities is low. Cases of IE caused by oral bacteria probably result from the exposures to low inocula of bacteria in the bloodstream resulting from routine daily activities and not from a dental procedure. Additionally, the vast majority of patients with IE have not had a dental procedure within 2 weeks prior to the onset of symptoms of IE.2-4 The role of duration of bacteremia on the risk of acquisition of IE is uncertain.45, 46 Early studies reported that sequential blood cultures were positive for up to ten minutes after tooth extraction and that the number of positive blood cultures dropped sharply after 10 to 30 minutes.24, 45-51 More recent studies support these data but report a small percentage of positive blood cultures from 30-60 minutes after tooth extraction.43, 52, 53 Intuitively, it seems logical to assume that the longer the duration of bacteremia the greater the risk of IE, but no published studies support this assumption. Given the preponderance of published data, there may not be a clinically significant difference in the frequency, nature, magnitude, and duration of bacteremia associated with a dental procedure compared with that resulting from routine daily activities. Accordingly, it is inconsistent to recommend prophylaxis of IE for dental procedures but not for, for example, commbivent 2.

Expanding DOTS and improving the existing DOTS strategy is the priority for national TB programmes. However, intensified case-finding for TB, used successfully on a wide scale and with appropriate emphasis, has the potential to increase case-detection rates and improve TB control. Generally, research should target the following. 1. Performing prevalence surveys. National prevalence surveys would help to define the burden of prevalent TB and provide a basis for more accurately estimating case-detection rates and time trends. Further, measuring HIV prevalence in the same surveys enables important questions on the impact of HIV on TB prevalence to be addressed. 2. Defining the threshold, if any, for starting intensified case-finding activities for national tuberculosis programmes and national HIV AIDS control programmes. Targeted intensified case-finding is recommended in populations with a high prevalence of HIV see the Interim policy on collaborative TB HIV activities ; or in congregate populations such as prisoners and miners irrespective of their HIV status. The epidemiological interaction between the HIV prevalence and the TB incidence needs to be understood better to define the best threshold of TB incidence or prevalence ; and HIV prevalence for starting intensified case-finding activities. 3. Improving case-detection strategies in clinical settings and where there is unmet need. TB case-finding among people living with HIV AIDS in clinics and hospitals, as well as household contacts, populations especially vulnerable to HIV infection and congregate settings should be intensified by improving testing algorithms and developing models to deliver this intervention. In particular, operational aspects of delivering intensified case-finding through existing community structures shops, churches, schools, community-based and home-based care volunteers ; should be better investigated. Further, operational research on the feasibility of integrating intensified case-finding with outreach activities, such as needleexchange programmes, should be a priority in the countries of the former Soviet Union and countries in South-East Asia, where epidemics of injecting drug use, TB and HIV are closely linked. Collaboration between the national TB and.
