THE RELATION BETWEEN SLEEP HYGIENE BEHAVIORS AND SEVERITY OF SLEEP DISORDERED BREATHING Graci GM, 1 Stepanski EJ1 1 ; Rush-Presbyterian-St. Luke's Medical Center, Introduction: Poor sleep hygiene has been widely cited as a cause of poor sleep in patients with insomnia. However, patients with other sleep disorders also report engaging in poor sleep hygiene behaviors. Therefore, it is possible that poor sleep hygiene can be a consequence, rather than a cause, of disturbed sleep. The purpose of this study was to assess the presence of poor sleep hygiene behaviors in a sample of patients with obstructive sleep apnea syndrome OSAS ; . Methods: All patients completed a measure of 7 facets of sleep behaviors as measured by a sleep hygiene behavior checklist. This scale used a 7-point Likert scale to rate the following behaviors: watching television, reading, listening to the radio, eating, talking on the telephone, working or studying, and using the computer in the bedroom. Scores ranged from 1 every night ; to 7 never ; , with lower scores representing poorer sleep hygiene. This instrument was administered consecutively to patients in our sleep disorders center. Patients identified as having sleep disordered breathing were retrospectively reviewed. Results: As expected, the group with an AHI 20 also had worse sleep, as shown by shorter TST p .0001 ; , increased % stage one sleep p .0001 ; , and increased WASO p .0001.
Do concur with your doctor and follow his directions completely when you are taking generic clomipramine. Used to relieve mental depression; one form of this medicine imipramine ; is also used to treat enuresis in children. Another form clomipramine ; is used to treat obsessive-compulsive disorders. Tricyclic antidepressants may be used for other conditions as determined by a doctor. Medline Plus. Risk can be computed as a multiple of the risk of an individual with average BMD. Consider, for example, a woman aged 70 years at the threshold of osteoporosis with a T-score of 2.5 SD. The gradient of risk for each SD decrease in BMD is 2.78 for hip fracture Table 19 ; , 1.8 for vertebral fracture and 1.6 for a humeral fracture see Table 18 ; . For a 70-year-old woman with an average BMD Z-score 0 SD ; , the T-score is approximately 1.55 SD, which is 0.95 SD above the threshold for osteoporosis. The risk of hip fracture for a 70-year-old woman with a T-score of 2.5 SD compared with a woman of the same age and average BMD is 2.780.95 2.64, the risk ratio for a vertebral fracture is 1.800.95 1.75 and the risk ratio for a humeral fracture is 1.600.95 1.56. BMD is normally distributed in a population at a given age, whereas there is an exponential relationship between BMD and fracture risk.3 Hence, individuals with an average BMD have a lower than average risk of fracture.72 Conversely, the average fracture risk is found in individuals with a lower than average BMD. For this reason, our calculations of fracture risk at any given T-score were adjusted accordingly. Fracture risk in men and women with an average BMD is given in Table 20, for example, clomipramine alcohol. Be predominantly dopaminergically mediated, as evidenced by the well-documented clinical response to haloperidol and other dopamine antagonists 58 ; , by the exacerbation with L-dopa and central nervous system stimulants such as amphetamines ; 59, 60 ; , and reports of lower CSF levels of the dopamine metabolite homovanillic acid HVA ; 61 ; . Moreover, OCD patients with comorbid tic disorder or GTS are usually resistant to conventional pharmacotherapy with proserotonergic compounds, and may benefit from adjuvant treatment with dopamine DA ; or DA 5-HT blockers 6, 55, 56, ; . This body of evidence suggests that there is an involvement of DA in least some OCD patients. With respect to the peripheral markers of the DA transmission, normal CSF levels of HVA have been reported 19, 64 ; , whereas the administration of fenfluramine produced increased inhibition of HVA secretion 33, 65 ; . The DA involvement has been assessed by measures of growth hormone response to apomorphine 66, 67 ; , and challenge with d-amphetamine 68 ; and methylphenidate 69 ; , with conflicting results. The serotonin and dopamine systems interact extensively, particularly in the basal ganglia 31 ; , an area that has been implicated in the pathogenesis of obsessive-compulsive phenomenology by several studies 1, 7072 ; . Indirect support for the involvement of both transmitter systems includes the observation of the emergence of de novo OC symptoms in patients on clozapine or risperidone, atypical antipsychotics with both D2 and 5-HT2 blocking properties 7375 ; , together with the demonstrated antidopaminergic activity of two antiobsessional agents, clomipramine and fluoxetine 76, 77 ; . Other Neurobiological Hypotheses Alternative neurobiological mechanisms have been proposed for OCD but they are in need of further confirmation. As already reported, functional neuroimaging studies have demonstrated dysfunction in the orbitofrontal cortex, basal ganglia and striatum, which normalize with successful treatment 78, 79 ; . Neuroendocrine mechanisms were implicated in the pathogenesis of obsessions and compulsions, based on studies employing oxytocin, vasopressin, and somatostatin 64, 8082 ; . These studies also need further replication. The Autoimmune Hypothesis Allen, Leonard, and Swedo first proposed the intriguing autoimmune hypothesis of OCD 83 ; after a thoughtful review of the literature. An association was drawn between infection with group A -hemolytic Streptococcus as well as other agents, including viruses ; , and the onset or the exacerbation of OCD in some children. The observation of an association between Sydenham's chorea an involuntary.
